Who Is Florida Blue?

Florida Blue is a member company of the Blue Cross Blue Shield Association. It is owned by GuideWell Mutual Holding Corporation, a mutual holding company created in 2013. Florida Blue sells health coverage for people of all ages, but its Medicare Advantage plans are only sold to people who live in Florida.

GuideWell

Jacksonville-based holding company

Florida Blue is a Florida-based company, which is different from national carriers such as Humana, UnitedHealthcare and Wellcare that sell plans in many states. Because Florida Blue only works in one state, its plans are built around Florida doctors and hospitals. That can be a plus if you live in Florida all year. It may be a drawback if you spend long stretches out of state, since most HMO plans only cover emergency and urgent care outside the service area.

Types of Florida Blue Medicare Advantage Plans

Florida Blue offers several kinds of Medicare Advantage plans. Not every type is sold in every county, so your choices depend on your ZIP code.

Plan type Example plan names How it works
HMO BlueMedicare Premier (HMO), BlueMedicare Classic (HMO) Use in-network doctors and hospitals except for emergencies and urgent care
PPO BlueMedicare Patriot Plus (PPO), BlueMedicare Value (PPO) In-network care costs less, but you can go out of network for a higher cost
C-SNP BlueMedicare Focus (HMO C-SNP) For people with certain chronic or disabling conditions
D-SNP Dual-eligible plans in some areas For people who have both Medicare and Medicaid

Plan names come from Florida Blue and Medicare plan listings for 2026 and 2027. Not every plan is sold in every county, and plan names can change from year to year.

If you qualify for both Medicare and Medicaid, or if you have a chronic condition, a special needs plan may give you extra care coordination. See our guide to Medicare Advantage special needs plans in Florida.

Florida Blue Medicare Star Ratings for 2027

Medicare rates each Medicare Advantage contract from 1 to 5 stars based on quality, member experience and customer service. Florida Blue has more than one contract, and each has its own rating.

3.5 stars

Florida Blue HMO contract (H1035) for 2027

3 stars

Florida Blue PPO contract (H5434) for 2027

Keep in mind: None of Florida Blue’s main contracts reached 4 stars for 2027. Star ratings are only one factor, but they can help you compare quality between carriers in your county.

Plans with 4 stars or more get quality bonus payments from Medicare, which they often use for richer benefits. Both Florida Blue contracts are below that level for 2027, so a lower rating may affect what a plan can offer in future years. The plan’s star rating is shown next to each plan in Medicare Plan Finder, and you can click into it to see scores for things like customer service, drug safety and how well the plan keeps members healthy.

What Florida Blue Plans May Include in 2027

All Medicare Advantage plans must cover the same Part A and Part B services as Original Medicare. Most Florida Blue plans also add extra benefits. Extras may include the items below, depending on the plan and area. Not all plans offer all benefits.

Possible extra benefits

✔

Dental coverage, with some plans offering a yearly dental allowance

✔

Vision exams and an allowance for glasses or contacts

✔

Hearing exams and copays for hearing aids

✔

An over-the-counter allowance loaded on a Blue Dollars prepaid card

✔

A SilverSneakers fitness membership

✔

HealthyBlue Rewards for completing certain preventive care

✔

A Part B premium giveback on some plans

2027 sample plan What Medicare listings show
BlueMedicare Premier (HMO) in Broward County $0 plan premium and a $4,451 in-network out-of-pocket limit
BlueMedicare Premier (HMO) in Southwest Florida Up to $3,000 a year for dental and a $45 quarterly OTC allowance
BlueMedicare Patriot Plus (PPO) $0 plan premium, $200 Part B giveback, no Part D drug coverage
BlueMedicare Focus (HMO C-SNP) $0 plan premium and a $130 Part B giveback in five counties

Based on 2027 Summary of Benefits and Medicare plan listings. Amounts differ by plan and county. You still pay your Part B premium, which is $202.90 a month in 2026.

Costs to Expect With a Florida Blue Plan

Many Florida Blue Medicare Advantage plans have a $0 monthly premium, but you still pay your Part B premium. You also pay copays or coinsurance when you get care, up to the plan’s yearly out-of-pocket limit.

Costs to compare

✔

Monthly plan premium, if any

✔

Medical deductible and drug deductible

✔

Copays for doctor, specialist and hospital visits

✔

The in-network out-of-pocket limit

✔

Drug costs, which are capped at $2,400 a year in 2027 for Part D drugs

The Part B premium is $202.90 a month in 2026. The Medicare Trustees project $209.50 for 2027, but CMS has not yet announced the final amount. Some plans include a Part B giveback that lowers what you pay each month.

A $0 premium plan is not always the cheapest choice over a full year. If you see specialists often, have a planned surgery or take several brand-name drugs, a plan with a small premium but lower copays may cost less in total. Add up the premium, your likely copays and your drug costs for the year, and compare that number across two or three plans. The out-of-pocket limit is the most you would pay for covered medical care in a bad year, so it is worth weighing as well.

Florida Blue HMO vs. PPO: Which Fits You?

The biggest choice for most people is between an HMO and a PPO. Both cover the same Medicare services, but they handle networks differently.

BlueMedicare HMO

✔

Usually lower costs

✔

Must use in-network providers for routine care

✔

Often includes Part D drug coverage

✔

Best if your doctors are in the network

BlueMedicare PPO

✔

Can see out-of-network providers for a higher cost

✔

No need to stay in the network for covered care

✔

Some PPOs do not include Part D

✔

Best if you want more provider choice

Tip: If you choose a plan without drug coverage, such as BlueMedicare Patriot Plus, make sure you have other creditable drug coverage, like VA benefits, to avoid a late enrollment penalty.

Snowbirds should look closely at network rules. If you split the year between Florida and another state, an HMO may leave you paying full price for routine care up north. A PPO gives more room to see providers away from home, though you will usually pay more for out-of-network visits. Ask each doctor you see often whether they accept the exact plan you are thinking about, not just Florida Blue in general.

How to Compare and Enroll for 2027

Florida has a crowded Medicare Advantage market, and the number of plans is shrinking for 2027. A review of CMS data found that Florida plan options fell from 611 in 2026 to 560 in 2027. That makes it important to check that your current plan is still offered.

1

Read your Annual Notice of Change It lists what is changing in your plan for 2027.

2

Check your doctors and drugs Make sure your providers are in network and your drugs are on the formulary.

3

Compare total costs Look at premiums, copays and the out-of-pocket limit together.

4

Enroll during AEP The Annual Enrollment Period runs October 15 to December 7, 2026.

If you would rather keep Original Medicare, Florida Blue also sells Medicare Supplement plans. Learn more in our guide to Florida Blue Medicare Supplement Plan G. You can also review the Medicare Annual Enrollment Period rules.
