➤ What is a Medicare Advantage SNP?
A Special Needs Plan, or SNP, is a Medicare Advantage plan designed for people with specific needs, such as those with both Medicare and Medicaid, certain chronic conditions or who live in an institution. Benefits, provider networks and drug coverage are tailored to that group.
➤ Who qualifies for a D-SNP?
People who have both Medicare and Medicaid generally qualify for a Dual Eligible Special Needs Plan, or D-SNP. Eligibility rules depend on your Medicaid level and state, so confirm your Medicaid status and check which D-SNPs serve your county.
➤ What is a C-SNP?
A Chronic Condition Special Needs Plan, or C-SNP, is for people with certain chronic conditions, such as diabetes, chronic heart failure or cardiovascular disorders. The plan usually verifies your diagnosis with your doctor, and its care management focuses on that condition.
➤ When can I enroll in a Medicare Advantage plan for 2027?
Annual Enrollment runs October 15 to December 7, 2026, with coverage starting January 1, 2027. Many SNP-eligible people also qualify for Special Enrollment Periods, so you may be able to change plans at other times of the year too.
➤ What happens if my Medicare Advantage plan is ending?
You will receive a non-renewal notice, and your coverage continues through December 31, 2026. You can choose another SNP or plan during Annual Enrollment or a Special Enrollment Period, and you generally have a guaranteed issue right to buy certain Medigap plans if you return to Original Medicare.
➤ Are insurers adding Special Needs Plans for 2027?
Yes. UnitedHealthcare is adding 40 Special Needs Plans and Aetna is adding 29 C-SNPs for 2027, while Anthem is expanding C-SNPs into Colorado and Maine. Some insurers, including Molina, Horizon BCBS New Jersey, Elderplan and Presbyterian Health Plan, are offering only SNPs for 2027.
➤ What is an I-SNP?
An Institutional Special Needs Plan, or I-SNP, is for people who live in a nursing home or similar facility, or who need that level of care at home. These plans coordinate care with the facility, so check which facilities work with the plan before enrolling.
